If you would like to obtain a copy of your medical records, either full or part of your records including copies of previous sick notes or consultations, you must complete the below subject access request form.
Once this has been completed, it will be passed to the secretaries who have up to 30 days to fulfil this request. Subject Access Request Form (DOCX, 14KB)
