How to Register
Anyone in England can register with a GP Surgery. At the Crown Medical Centre we have a large catchment area covering most of Taunton as well as some of the surrounding area. Reception can advise whether you live within the area or not or you can use our postcode checker below.
To register with the surgery, you can download and complete the registration forms below or pick some up from reception. We ask that patients provide a proof of address for example a utility bill and photographic ID when registering but this is not essential. You do not need to provide an NHS number to register. If you do not have photographic ID or a utility bill, please let our reception team know. Once we have registered you we can request your medical notes from your current surgery. Please note this can take up to 48 hours. Each patient is registered with a "Named GP" for continuity of care. To find out yours, please ask at reception.
Our new patient packs are as follows. You must complete the registration pack in full. You are able to email the pack back to the surgery via our generic email account; email@example.com

If you are staying away from home need urgent GP care about a new issue, you are able to complete the below form and ring the surgery to request an appointment. Please be aware we only see temporary patients are staying within our practice boundary, please use the Post Code checker below to see if your temporary address is within our catchment area. If you are staying in the area and have ran out of your regular medications, you are able to contact your usual GP surgery and ask them to organise an electronic prescription to be sent to a pharmacy near where you are staying. Please note you must be registered at a GP surgery in England to use this service.
